Diabetes Medical Management Plan

Safe at School® is pleased to offer its updated Diabetes Medical Management Plan (DMMP) for use by schools and pediatric diabetes health care providers nationwide. This new fillable form will allow for a more standardized and efficient completion of student diabetes care orders through expanded options for prescribers to tailor care instructions to the individual student, providing greater clarity for those charged with the care of children with diabetes in the school setting. Safe at School ® encourages school nurses, diabetes health care providers, and public health staff to become familiar with how to use this form through training and education.

The DMMP should be updated annually or whenever the child's regimen, level of self-management, or school circumstances change. Parents/guardians are responsible for getting their child’s completed and signed DMMP (or doctor's orders) from their child's diabetes care provider. They should then give the DMMP to their child's school to implement and carry out doctor's orders. It should explain all the necessary elements of care and assistance needed at school.
